Amerelle Wallplates - Cottage - Triple Rocker Wallplate in White
As noted by others, these tend to separate when tighter even with limited torque. I applied epoxy around the inside seam and it seemed to help. Be sure to lightly tighten the screws. Other than that, they look better than the standard plastic wall plates.
